Building Strong Primary Care in Underserved Communities
The innovative Bank of American Loan Repayment Program helps physicians and nurse practitioners who have desire to work in community health centers. The Kraft Family National Center for Leadership and Training in Community Health will improve access to and quality of healthcare for the neediest people in our communities.
Strengthening Community Health Centers: Care in the Right Place at the Right Time
In 2010, Partners and North Shore Medical Center committed $3 million to expand Lynn Community Health Center. The expansion includes the relocation of an existing Walk-In Clinic; the centralization of patient registration and enrollment services to provide better service to patients accessing Medicaid and other public insurance programs; and the creation of a second story and basement shell space to be developed in the future.
Connecting Patients with Primary Care
Partners is working to ensure that patients get the right care at the right place. Since October 2005, our primary care access projects work to help patients who come through our emergency and other hospital departments connect with community-based primary care providers who ensure they receive ongoing, comprehensive care.
