The Partners Center for Connected Health develops innovative and effective solutions for delivering quality patient care outside of the traditional medical setting. The Center engages in pioneering research in a wide range of connected health-related areas and works to advance the field through its convening and publishing activities.
About Our Programs
Our Center for Connected Health programs use a combination of remote-monitoring, wireless, and online communications and intelligence to improve patient adherence, engagement, and clinical outcomes. Participants in our programs are patients and providers at Partners-affiliated practices and hospitals throughout New England.
In addition the Center’s conferences bring more than 1,000 health care professionals each year to Boston to discuss and debate the means of moving health care out of the clinical setting and into the day-to-day lives of those who need care.

Telehealth, mobile health (mhealth), remote care, and disease management initiatives reflect the opportunities for technology-enabled care programs.
